Femoral neck fracture

Ask during the case

  1. On the films before incision: "Which Garden type is this, nondisplaced or displaced, and which two things on the film told you?"
  2. Opening the capsule (or placing the guide pins): "Where does the blood supply to the head run, and how close are we to it right now?"
  3. Closing: "Which nerve did today's approach put at risk, and what will you check in recovery to show it's working?"

Point at: the head-neck junction on the fluoroscopy screen or in the wound, and the posterosuperior neck where the retinacular vessels run, while they name them.

End of day: "Draw me an AP hip, put in the four Garden patterns, then draw the head's blood supply from behind." Two minutes each, no notes.

Where this kit and your technique differ, yours wins.

The night before. They prep alone, redraw at turnover, and get a cold check tomorrow morning. They won't see these questions.
